AS OF sep 2024 - new forms

All New Clients will receive an email and/or text notification once their appointment is requested / scheduled with the applicable required forms attached as a link associated with Square Contract Forms. New Clients will be required to provide a valid email address.

Forms will automatically be attached to emails and text notifications when mobile number provided. Forms can be completed via mobile device, but are best viewed using a larger screen.

New Clients will Initial and sign the applicable forms by "typing" using the keyboard on whatever device they are using.
All applicable forms will be initialed, signed, and submitted digitally and kept on record securely.
Copies of all completed applicable forms will automatically be emailed and texted to New Clients for their records.
